Therapeutic Effects of Electrical Stimulation on Ankle Spasticity in Individuals with Stroke: A Meta-Analysis

電刺激對中風患者踝關節痙攣之療效:統合分析研究

背景和目的:本研究利用統合分析方法來驗證電刺激對中風患者踝蹠屈肌痙攣之療效。方法:本研究搜尋了PubMed、Cochrane library、MEDINE、PEDro、台灣碩博士論文系統等資料庫,於2003年至2013年期間比較接受電刺激和無電刺激治療的痙攣量測相關文獻,主要測量包含改良版Ashworth量表(modified Ashworth scale)和H/M比值。結果:共收錄4篇文獻進行統合分析,根據改良版Ashworth量表(95% CI:-0.67~-0.06, p=0.02)和H/M比值(95% CI:-0.2~-0.05, p=0.0006)的統合分析結果顯示接受電刺激治療後會顯著改善踝蹠屈肌痙攣之現象。結論:本研究證實電刺激有助於改善中風患者踝蹠屈肌痙攣之現象。

Background and Purpose: We performed a meta-analysis and systematic review to investigate the therapeutic effects of electrical stimulation (ES) on ankle plantarflexors spasticity in individuals with stroke. Methods: We searched PubMed, Cochrane library, MEDINE, PEDro, Chinese Electronic Periodical Services and National Digital Library of Theses and Dissertations in Taiwan until December 2013 and identified studies that compared spasticity measurements after receiving ES with non-receiving ES. Outcome measures included modified Ashworth scale (MAS) and H/M ratio. Results: A total of 4 studies were identified for meta-analysis. Meta-analysis revealed improvement in ankle plantarflexor spasticity indicated by MAS (95% CI: -0.67~-0.06, p=0.02) and H/M ratio (95% CI: -0.2~-0.05, p=0.0006) after receiving ES. Conclusions: This study demonstrated that ES significantly decreased ankle plantarflexors spasticity in individuals with stroke.
