體感遊戲對於亞急性中風患者平衡之療效:統合分析與系統性回顧

Effects of Exergame on Balance in People with Subacute Stroke: A Meta-Analysis and Systematic Review

背景和目的:中風後有許多因素都會導致平衡功能障礙,使中風患者無法安全的執行日常生活活動,而影響其獨立生活能力。平衡運動是中風復健的重要治療項目之一,也是亞急性期的治療重點。體感遊戲是一種結合遊戲與身體動作的一種運動形式,提供使用者多重感官刺激,具有高度推廣性、低成本的優點,對於亞急性或慢性期中風患者都是有效果且安全的介入方式。本研究採用統合分析與系統性回顧以探討體感遊戲介入對於亞急性中風患者平衡能力的改善效果。方法:本研究搜尋相關電子資料庫,納入2009年1月至2019年12月的隨機對照試驗。使用皮卓量表與2011年版牛津實證醫學中心證據等級分析納入文獻的品質與建議等級。若有兩篇以上文獻使用相同的平衡測試方法,則使用Review Manager Version 5.3軟體進行統合分析。結果:本研究納入6篇文獻進行統合分析和文獻回顧,統合分析結果顯示,亞急性期中風患者經過體感遊戲介入後,於計時起走測驗(mean difference [MD] = -0.41, 95% confidence interval [CI]: -0.79 ~ -0.03, p = 0.03)、伯格氏平衡量表(MD = 3.5, 95% CI: 0.56 ~ 6.46, p = 0.02)、功能性伸取測驗(MD = 2.89, 95% CI: 0.36 ~ 5.42, p = 0.03)皆有顯著的改善效果。根據系統性文獻回顧可知,體感遊戲亦有降低跌倒風險與改善日常生活活動和生活品質的效果。結論:體感遊戲對於亞急性中風患者的平衡表現有顯著性的改善效果。

Background and Purpose: Balance is affected by many factors in individuals with stroke. Balance dysfunction makes individuals impossible to perform activities of daily living safely. Balance training is a critical component of rehabilitation in subacute stroke. Exergame is the activity of playing video games that involves physical exertion and is thought of as a form of exercise. Exergame provides the subject with multisensory feedback and can be controlled and modulated in clinical and home settings. Previous studies have demonstrated exergames as a beneficial, cost-effective, and safe intervention for those with subacute stroke. The purpose of this study was to use a meta-analytical approach to assess the effects of exergame on balance and systematically review the evidence of exergame in people with subacute stroke. Methods: A systematic search of the randomized controlled trials from January 1, 2009 to December 31, 2019 was conducted. Some related databases were searched. The quality of eligible studies was evaluated using PEDro Scale and Oxford classification. Meta-analysis was conducted using Review Manager Version 5.3 software. Results: Six studies were included in meta-analysis and systematic review. The results of meta-analysis showed that exergame significantly improved the Timed Up and Go test (mean difference [MD] = -0.41, 95% confidence interval [CI]: -0.79 ~ -0.03, p = 0.03), Berg Balance Scale (MD = 3.51, 95% CI: 0.56 ~ 6.46, p = 0.02), and Functional Reach test (MD = 2.89, 95% CI: 0.36 ~ 5.42, p = 0.03) in people with subacute stroke. According to systematic review, exergame also decreased fall risk and improved the ability to perform activities of daily living and quality of life. Conclusion: Exergame can improve balance performance in people with subacute stroke.
