本研究旨在編製一份具臨床應用價值的國語聲調辨識測驗表單,並進行信度和效度分析;同時將此測驗應用於評估感音神經性聽力損失(sensorineural hearing loss, SNHL)成人的聲調辨識表現,以正常聽力成人為對照組,驗證測驗表單的區辨效度。國語聲調辨識測驗表單共有兩個完整的A、B全表,以及四個半表A1、A2、B1、B2。受試對象分為三組:正常聽力成人組(聽常組)、未佩戴聽覺輔具的SNHL成人組(聽損組I)和已佩戴聽覺輔具的SNHL成人組(聽損組II)。所有受試者均在標準化聽力檢查室中,個別接受中耳功能檢查、純音/語音聽力檢查及聲調辨識測驗。研究結果顯示,所編製的測驗表單具有良好的內容效度(專家評比題項語料的口語熟悉度)、效標效度(測驗表單與音節聲調聽辨測驗得分之間的相關係數介於0.88至0.92,呈高度正相關)及區辨效度(聽常組與聽損組I之間存在顯著差異),亦具有高度的折半信度(0.94~0.99)、複本信度(0.94~0.99)及重測信度(0.88~0.99)。三組別在國語聲調辨識測驗表單(全表及各半表)和音節聲調聽辨測驗的得分,聽常組的平均正確率分別為100%和98.27%,聽損組I為85.45%~86.91%和83.55%,以及聽損組II為70.40%~74.40%和75.20%。進一步比較聽損組I和聽常組,統計檢定結果顯示,未佩戴聽覺輔具的SNHL成人在聲調聽覺辨識表現顯著低於正常聽力者(p < .01)。綜合而言,本研究編製的國語聲調辨識測驗表單,具備良好的信效度,適用於評估SNHL成人的聲調辨識能力,具有臨床應用價值,可做為聽力評量的參考工具。
The study aimed to develop the Mandarin Tone Recognition Test (MTRT) Form and conduct reliability and validity analyses for clinical assessment. Additionally, it sought to apply this test form to evaluate the performance of adults with sensorineural hearing loss (SNHL) in the MTRT and compare their results with those of normal hearing (NH) listeners to establish the discriminant validity of the MTRT. The test comprised two full tables (A and B) along with their corresponding semi-tables (A1, A2, B1, and B2). The subjects were categorized into three groups: the NH group, SNHL adults without hearing devices (HL I), and SNHL adults with hearing aids or cochlear implants (HL II). All subjects underwent standardized testing procedures in a controlled audiology booth, utilizing tympanometry, an audiometer, and speech perception tests. The main results showed that the MTRT demonstrated good content validity, criterion validity (ρ = .88 to .92), and discriminant validity (significant differences between NH and HL listeners). The test also exhibited high alternate-forms reliability (ρ = .94 to .99), split-half reliability (ρ = .94 to .99), and test-retest reliability (ρ = .88 to .99). Furthermore, SNHL listeners performed significantly worse than NH listeners in recognizing the four Mandarin tones in both MTRT (HL I: 85.45% to 86.91%; HL II: 70.40% to 74.40%; NH: 100%) and the Mandarin Lexical Tone Recognition Test (MLTR) (HL I: 83.55%; HL II: 75.20%; NH: 98.27%). In conclusion, the MTRT demonstrated strong reliability and validity, making it an effective tool for assessing tone perception ability in adults with SNHL and valuable for clinical hearing assessment.
