目標:本研究之主要目的為:探討醫院不同員工屬性的倫理氣候類型、探討醫院員工及病人對醫病關係感受的差異、探討醫院倫理氣候和醫病關係的相關性。方法:本研究以倫理氣候量表及醫病關係量表為測量工具,針對台北市某一區域醫院的員工及病人發出200份問卷,回收率85%。結果:分析結果顯示,醫師和行政人員的倫理氣候一致性顯著的高於護理人員及其他醫療人員;在醫病關係良窳的認知上,滿分5分,全體院內員工平均得分為3.66,住院病人平均為4.45。主治醫師得分最高為3.94,行政人員最低為3.33;關懷和自我導向的倫理氣候和醫病關係最為正相關。結論:研究結果顯示同一醫院不同醫療專業間的倫理氣候一致性確有差異、直接照護病人者對醫病關係的感受較接近病人的感受、需要再加強同理心方可促進醫病關係和諧。
Objectives: The purposes of this study are: whether the ethical climates of different professions in one healthcare facility differ; how healthcare workers feel differently from their patients in terms of healthcare provider-patient relation; the relationship between ethical climates and healthcare provider-patient relation. Methods: This study applied Ethical Climate Questionnaire and self-developed healthcare provider-patient relation questionnaire to a regional hospital located in Taipei. Two hundred people including hospital staff and inpatients were surveyed with an 85% response rate. Results: Physicians and the administrative staff have significantly stronger ethical climates. On the scale of five, all hospital personnel average 3.66 in healthcare provider-patient relation assessment as compared to 4.45 for inpatients. Physicians score the highest 3.94 and the administrative staff the lowest 3.33 among hospital staff. Caring and egotism are two ethical climate aspects most positively correlated to healthcare provider-patient relation. Conclusions: Different professions do differ on their ethical climate propensities. Immediate caregivers can better understand how their patients feel in terms of healthcare provider-patient relation. Empathy needs further attention in the promotion of healthcare provider-patient relation.
