從管理主義的角度看台灣醫療環境之變革與發展

摘要

本研究針對10位受訪者進行深度訪談，其中醫院高階主管4位、社會工作部門主管6位。研究發現臺灣醫療環境之變革與發展之內涵有全民健康保險制度增強嚴格給付暨管理機制、醫院評鑑下醫院間市場競爭效應、醫院走向企業經營管理模式、醫院管理採用管理主義運作使得衛生醫療環境之醫院服務有質變等現象。故本研究建議簡化評鑑形式主義、拉近醫院管理理念與醫療專業主體的立場、引導不同屬性醫院發揮不同且適切的社會功能。

This study conducted in-depth interview on ten subjects, including 4 high-rank directors in hospital and 6 directors of social service department. According to findings of this study, (1) strict payment and management of national health insurance system; (2) market competition effect among hospitals in hospital evaluation; (3) managerial model of corporate operation of hospital; (4) hospital management adopts managerialism and thus hospital service of health care and medical environment shows qualitative change. Thus, suggestions of this study are below (1) to simplifies formalism of evaluation; (2) to reduce the gap between business management concept of hospital and professional medical subjects; (3) to result in different kind of hospitals to perform their social functions.
