我的美麗與哀愁-醫院社會工作才能的省思

REFLECTIVE PRACTICE IN HOSPITAL SOCIAL WORK COMPETENCE IN TAIWAN

摘要


社工人員運用社會工作知識與技術於醫院,協助病患或家屬排除醫療過程障礙,使因疾病而產生之各種社會與家庭問題得以解決。然而,醫院社工師在職場的角色定位不清、缺乏專業自信、專業認同低落、對工作認知產生衝突感等經常被討論,本研究目的要探究醫院社會工作才能與困境,研究設計以質性深度訪談法收集資料,邀請醫院社工師參與。研究發現醫院社會工作才能多元廣泛展現於兩部分:一是協助各種疾病患者的個案工作,包括病患關懷、疾病適應、經濟補助、出院準備等;其二則是醫院分派之任務。醫院社會工作才能的多樣性是優點亦是限制,醫院管理階層將社工師放在有利於醫院經營的位置,而非以病患利益的考量,因此排擠以病患中心的個案工作,影響醫護人員看待社會工作的功能及社工師自我的專業認同。建議思考醫院社會工作才能專業重構,即醫院社會工作者調整過去根深蒂固的工作邏輯,增加其專業技能並創新其關鍵角色在跨專業團隊、問題導向的短期處遇、經濟補助評估制度、運用團體工作、參與出院計畫。

The role of hospital social workers is criticized, which lack of confidence, low professional identity, and a sense of conflict in job. The purpose of studying is to explore the social work competence in the hospital. The studying conducts qualitative research and depth interview to collect data that invited 10 hospital social workers to participate. The findings that the social work competence in the hospital can be shown in two parts: one is to assist patients cope with various illness, including patient support, illness adaptation, financial assistance, and discharge plan. The other is assigned by the hospital management. The social work competence in the hospital are in multiple dimension which strength is social work solve psychosocial problems for patients, and limitation is hospital management assign social work tasks benefit to hospital marketing, rather than concern the interests of patients. It could impact the patient-centric casework and professional identity for social workers in the hospital. The suggestion is that social work competence would be professional reconstruction in the hospital.
