以糖尿病為例探討影響醫院建置個人健康記錄入口網站之關鍵因素－資訊人員之觀點
Critical Factors Influencing the Implementation of Personal Health Record Portals for Diabetes Mellitus-Perspectives of Information Systems Departments
摘要
隨著病患對自我健康資訊關心程度增加及院際間電子病歷交換需求，個人健康記錄（Personal Health Records，PHR）應為一可行解決方案。本論文結合「入口網站」與PHR概念，以國內十大死因慢性疾病－糖尿病為主要內容，針對醫院資訊主管和資深資訊人員進行調查，本論文模式整合創新擴散理論與科技組織環境架構，包含組織、環境、資訊科技、資訊隱私權4個構面及13個可能影響因素。研究母體為國內參與健保局95年度第二季糖尿病醫療給付改善方案試辦計畫地區級以上醫院共計241家，回收有效問卷72份，有效回收率29.88%。經區別分析，結果顯示高階主管認知、支援配合、相容性、複雜性、相對優勢及適當存取等6個變數為醫院建置糖尿病PHR入口網站的主要因素。

With an increasing awareness of patients' health, and the need to integrate all health information among health institutions, the application of the Personal Health Records (PHR) is a viable approach. This research will use the portal platform, combining with the concept of PHR, to investigate the key factors which will affect the implementation of PHR Portal of diabetes mellitus (DM), which is one of the ten leading causes of death in Taiwan. This research combines Innovation Diffusion Theory and Technology-Organization-Environment to develop an integrated model. Organizational characteristics, environmental characteristics, technological characteristics, and characteristics of information privacy are included in the empirical study. A total of 241 questionnaires were mailed to chief information officers and senior MIS staffs of local hospitals, regional hospitals, and medical centers in Taiwan. 72 questionnaires were returned with a response rate of 29.88%. The results of discriminant analysis indicate that top management recognition, external support, compatibility, complexity, relative advantage, and proper access are important determinants of implementation of PHR portal of DM to hospitals.
