公立(營)機構績效不振因素分析兼論署立醫院突破困境因應之道

Causes of the Economy Depression on Public Hospitals and the Solutions

摘要


二十一世紀千禧年的到來,醫療服務業亦邁入新的紀元。伴隨著景氣的低迷及國家財政困難;公務預算逐年減少補助;公立醫院經營已面臨第一項難題。其次公立醫院受限於公務機關醫療、人事、會計審計、採購、政風….等法令束縛,組織僵化,管理經營上層層受限,造成其績效不彰,為人所詬病,實非國人所樂見。其三;健保財務吃緊;所申報之醫療費用易剔除率高;單靠這項收入來維持醫院營運難以達成。因此發展多角化業務;如將舊有建築改設置護理之家以解決老人及重症者安養問題;設立作月子中心以解決忙碌社會的產後護理問題;依法設立托嬰、托兒中心以解決教養問題….等;將0至100歲者均納入服務範圍,才能擴大醫院利基。因此如何吸取民問醫院靈活經營方法及善用公立醫院本身優勢,發展出適合現階段發展策略,才是公立醫院邁向永續經營所需思考的問題。我們期待公立醫院轉型改造成功,使他們成為高品質,高效率又肩負國家平衡醫療資源政策的醫院群,為更廣大鄉親提供最好的服務。

As the step of millennium in this 21st century is getting closer and closer, the medical service is stepping into a new generation too. Accompany with the atmosphere of the society and economy depression, the public fee is getting less and less as time goes by. Public hospitals are facing their first task. second those public hospitals are trapped inside the public medical system, personal manager, accounting, atmosphere of government, buying and selling, those rules of the government, the freeze organization, rules everywhere on the manager business, which leads the negative result of their running business. Third, the effect of the economy depression upon the health committee, the possibility of failures of those registered medical fee is so high; therefore it is very hard to carry on the medical service with so little money that they've made. Therefore, services towards to all directions started to come out, for example: decorate those old buildings and make those buildings a home of those homeless elderly and unhealthy elderly. Set up a center for pregnant women as one way to solve their health problems after they give birth. Also set up baby sitter centers to help those busy parents to educate their children. They provide service to the range aged between 0 to 100, therefore they could spread out the profit of public hospitals. To make full use of their techniques on medical service business and their strong points as public hospitals, and leads to the grows of their business which fits this age is what public hospitals need to think about. We are hoping that the changing on the style of public hospitals will be very successful, which leads to high standard, high efficiency and also carry the responsibility of balance the medical resources. to provide the best service to all human beings.
