急診病患轉送病房護理交班之改善方案

摘要


護理交班是臨床例行且重要的工作項目,因正確完整的交班,可讓病患得到持續性的照護。以病患為中心的交班內容,透過交班方式將轉送患者的資料傳遞給承接照護的單位,若輔以適當的工具,可增加交班的效率。急診病患轉送病房的次數頻繁,護理人員在急救處置穩定患者病況後與病房護理人員交班的正確完整性,關乎患者後續的治療及照護,經由現況調查瞭解本院急診病患轉送病房電話交班的遺漏缺失,針對問題提出解決方案,藉由設計“急診病思轉送病房交班單”的使用,從結果看出此工具的使用率達95%,不僅達到建立急診護理人員轉送病患電話交班規範的目的,也使電話交班內容完整性之改善達80%,同時也提昇病房護理人員對急診轉送病患電話交班的滿意度達60%。

Nursing report is very important. Because of complete patient's data report, can get continue nursing care. After collecting patient data, then use the adequate methods of transfer report and tools, you should increase nursing report efficacy. Transferring patients to wards is a daily routine work for emergency department. The ED nurses must prepare patient's data to hand over to the ward nurses after managing patients' life-threatening problems and stabilizing patients' conditions. A correct and complete patient's data may affect patients' continuing treatment and nursing care. According to the responses from the ward nurses, ED nurses' transfer reports were incomplete. So, a checklist of transfer port for ED patients was developed to assure continuous nursing care quality. The results were shown that the checklist was not only a guideline for giving telephone port, but also could improve the completion rate of the report.
