探討醫護人員對醫療及給藥錯誤告知之態度及障礙

Perceived Barriers And Attitude toward Disclosure of Medical and Medication Errors among Physicians and Nurses

摘要


目的:醫療及給藥錯誤一直都是全球關注的議題,許多國家皆有推行揭露政策,但告知病人與紀錄病歷對醫護人員而言是一大挑戰,因此本研究目的想探討醫護人員對於醫療及給藥錯誤告知及紀錄病歷之態度及障礙。方法:本研究採橫斷式相關性之研究設計,以立意取樣北部某醫學中心各病房及加護室不同職級之醫師及護理師,用結構式問卷自填方式收集資料。結果:總共回收有效問卷醫師118份及護理師133份。醫師及護理師在錯誤告知與記錄病歷之態度及障礙上,顯示害怕法律及訴訟問題。在「告知障礙」醫師顯著小於護理師。結論:法律訴訟問題為醫護人員在告知上最關心議題,應思考制定相關法律保障醫護人員,才有可能確實告知錯誤。

Objectives: Medical and medication errors are global issues. Although many countries have specific disclosure in healthcare, it is a big challenge for healthcare professionals to disclose medication and medical errors. This study aims to discuss perceived barriers and attitude toward disclosure of medical and medication errors among physicians and nurses. Methods: This is a cross-sectional study design. A purposive sampling approach was adopted to enroll physicians and nurses with different job ranks from different wards and intensive care units (ICU) in a medical center in Northern Taiwan. Results from a self-administered (structured) questionnaire were collected for data analysis. Results: Total of 118 and 133 effective questionnaires were recovered from physicians and nurses, respectively. The results showed that both physicians and nurses are greatly concerned about legal and litigation issues during the process of disclosing and recording, but obviously physicians have less "perceived barriers in disclosing errors" to patients than that of nurses. Conclusions: Legal and litigation issues are the most critical domains concerning healthcare professionals during the process of error disclosure. Therefore, it is recommended to stipulate relevant laws and regulations for protections of healthcare professionals to facilitate a transparent disclosure policy.
