目的:藉由分析急診病人三日內返診率之變化及影響返診相關因素,瞭解並探討在受到新冠肺炎(COVID-19)疫情影響下,急診就醫病人之差異及對於急診照護品質之影響,提供衛生主管機關掌握病人就醫行為及作為醫院管理部門擬定管理措施的參考。方法:本研究擷取2020年2月1日至4月15日急診就醫資料與2019年同期比較,共取得18,692筆急診就醫紀錄,以SPSS 19.0統計軟體進行描述性及推論性統計。結果:受到疫情影響,2020年該期間急診就診人次相較去年同期減少25.3%,病人三日內再返診率為4.24%,略低於2019年4.54%;三日內與前次疾病相關性急診返診率為1.84%,則較去年同期1.55%顯著增加。另由資料分析,2020年疫情期間,返診病人的性別、年齡、入院方式、檢傷級數、回診原因、上次急診後動向與去年同期返診病人有顯著差異。結論:新冠肺炎疫情造成醫療機構急診就醫人次明顯減少,因相同疾病診斷三日內急診返診率卻較前一年顯著增加情形,且前次急診未完成治療即離院的比率也較高,故建議急診單位在疫情期間應注意未完成治療即離院病人之照護,以減少返診之發生。
Objectives: Analyzing the change of emergency patients return visit rate within three days, and patient characteristics, to understand and explore the difference between patients and quality of emergency treatment care by the impact of the new coronavirus (COVID-19) epidemic. Methods: This study captures the emergency medical records from February 1, 2020 to April 15 in case hospitals, and compares with the data for the same period in 2019, obtaining a total of 18,692 emergency medical records, with SPSS 19.0 statistical software for descriptive and inferential statistics. Results: Affected by the outbreak, the number of emergency visits decreased by 25.3% in 2020 compared to the same period last year. The return visit rate within three days is 4.24%, which is slightly lower than the 4.54% in 2019; The rate of emergency return visits within three days and related to the previous disease is 1.84%, which is higher than 1.55% in the same period last year and has a significant difference. According to data analysis, during the 2020 epidemic, the gender, age, reasons for a return visit, the referral of the last emergency visit is significantly different from those patients in the same period last year. Conclusions: The outbreak of new coronavirus caused a significant decrease in emergency department inpatient numbers, and the return rate related to the previous disease within three days has a significant increase than last year. It is recommended that the emergency department should pay attention to the care of patients who leave the ED before treatment is completed during the epidemic to reduce the number of return visits.
