目的:瞭解精神病患主要照顧者需求的重要性與滿意程度之現況,並探討主要照顧者對照護需求重要性與滿意程度之差異,並以IPA(Importance-Performance Analysis)分析確認主要照顧者對照護需求優先提供服務的順序。方法:採結構性問卷為研究工具,以方便取樣收集資料,研究對象為北區某區域教學醫院精神科急、慢性病房、日間病房及門診,曾於精神科急性病房反覆住院達2次(含)病患之主要照顧者,共發放161份,有效問卷有150份,有效回收率為93%;以IPA分析確認主要照顧者需求優先提供服務的順序。結果:主要照顧者需求最重要項目為:「政府能提供經濟上的幫助」,滿意程度最高項目為:「醫護人員與您討論病人疾病症狀及發生的原因」,IPA分析中得知「實質性需求」中「政府能提供經濟的幫助」及「政府能提供病人工作訓練及工作機會」需改善。結論:本研究顯示主要照顧者社經地位不高且經濟及照顧負擔沉重,建議透由政府及健保給付減輕經濟的負擔,同時也減輕長期照顧的沉重壓力,讓主要照顧者選擇非醫療院所所設立之多元化社區機構及社區復健中心,亦將有助於政府長期照顧服務法之推動。
Objectives: This study aimed to understand the importance and satisfaction of the needs of mental patients' main caregivers. In addition, importance-performance analysis (IPA) was used to confirm the order in which priority care providers need to prioritize services. Methods: We used a structured questionnaire as a research tool. This study was conducted using a questionnaire survey in a regional teaching hospital in northern Taiwan. A total of 161 questionnaires were sent to the main caregivers of psychiatric patients, and 150 valid questionnaires were returned for a valid response rate of 93%. IPA was used to confirm the order in which caregivers should prioritize services in the future. Results: Findings indicated that the government providing financial aid for mental patients is the most critical need for mental patients' caregivers. Moreover, the highest of satisfaction was care staff discuss the symptoms of the patient and the causes of the disease with you. Thus, the substantive demand on the main caregiver was distinguished from IPA results, which indicated that the items of "the government can provide financial help" and "the government can provide patient job training and job opportunities" of substantive demand must still be improved. Conclusions: This study demonstrated that caregivers have a low socioeconomic status and heavy economic and care burdens, and it proposed reducing economic burden through government and health care payments. This would also relieve the heavy pressure of long-term care. Allowing caregivers to choose a diverse community agency and community rehabilitation center established by a nonmedical institution will also help the government promote its long-term care services law.
