儘管新的氧喘藥物不斷發展成功,氧喘病人的治療仍不如理想,其中病人對氧喘療法的依從率低下,醫病關系的不密切、不一致的履行治療指引,這都是重要的原因。在台灣衛生署於2000年亦編制一本屬於全國性的氧喘治療指引,因為醫師與病患對遵照指引的療法是控制氧喘成功的重要因素。在最近的研究報告中顯示,降低氧喘治療指引的利用,部份是歸因於對氧喘知識的不瞭解,為了要評估推行指引的功效、及有關氧喘概念及其正確療法的問卷分送于區域醫院、醫學中心、內兒科攻師、護理人具及開業醫師,結果發現遵從指引療法的履行率甚低,而且對指引內容的瞭解度不夠,很明顯的顯示醫師與護理人員對氧喘的再教育十分重要,藉以矯正他/她過於主觀的治療行為與觀念。因此我國氧喘病患並沒有獲得適當的病情評估及治療,亦是引起氧喘相關疾病罹患率及死亡率上升的部分因素。
Despite advances in drug treatment, outcomes of patients with asthma remain unsatisfactory. Low rates of patient compliance to the prescribed treatment regimen, inadequate physician-patient communication, and inconsistent implementation of evidence-base treatment guidelines are the most important causes. In Taiwan, a national asthma guideline was prepared for publication by the Health Department in 2000. Physician and patient adherence to the guidelines is an important factor the control of asthma. Recently in several articles, researchers have suggested that the under utilization of the asthma guidelines may in part be related to a lack of understanding. In an investigation of the effectiveness of these guidelines, a questionnaire was sent to the doctors practicing in the regional hospitals/medical centers and private clinics. In addition, the nursing groups in the hospitals were included. In conclusion, poor adherence and lack of understanding of the guidelines was found. It clearly indicated a need for further educational programs to the professional health care providers that cause their behavior and concepts of treatment to be too subjective. Consequently, asthma patients did not receive appropriated evaluation or treatment, which may be part of the reason for increasing the asthma-related mortality and morbidity rates in Taiwan.
