摘要

背景及目的：調查過敏性鼻炎患者接受半導體雷射(Diode)治療後之改善程度。
方法：追蹤88年9月至90年5月間有確定過敏原之過敏性鼻炎患者，以Allergy Outcome Survey (AOS)問卷調查其雷射手術後三個月、六個月及一年之症狀改善度及藥物使用量。共調查42位患者。
結果：所有患者皆無術後出血而須鼻部填塞止血的情形，回覆問卷有28位，但有效回覆者有27位（有效回覆率64.3%）。AOS中的Symptom Score在三個月、六個月及一年時有改善的比率為81%、74%、52%; 而Medication Score在三個月、六個月及一年時有改善的比率為67%、30%、11%, Total Score在三個月、六個月及一年時有改善的比率為78%、63%、44%。手術的效果隨時間之增加而消退。
結論：以接觸模式之半導體雷射治療過敏性鼻炎的效果，依據統計分析，在一年後仍會有症狀上的改善，但在藥物使用量及Total AOS Score上與術前相較已無統計上顯著之差異。

Background and purpose: The purpose of this study was to measure the outcome of treatment of allergic rhinitis with diode laser in the contact mode.
Methods: From September 1999 to May 2000, 42 consecutive patients under local anesthesia were treated with a continuous diode laser (wavelength, 805 nm, laser parameter, 12W) in ”contact” mode. All patients were refractory to conservative medical treatment. After a follow-up of 1 year, we measured the result of treatment using an Allergy Outcome Survey (AOS) questionnaire 3 months, 6 months, and 1 year postoperatively.
Results: A total of 27 of 42 patients (64.3%) completed the questionnaire. The mean operation time took 1 minute and 28 seconds. Nasal packing was unnecessary, and there were no observed complications (e.g., major bleeding). Statistical analysis revealed significant improvement of symptom score 1 year after laser surgery. A total of 44% of the patients had improved total scores 1 year after laser treatment. Nevertheless, these outcomes declined with time.
Conclusion: Diode laser treatment of allergic rhinitis is a useful procedure, which can be performed as outpatient surgery under local anesthesia. The short operation time and less discomfort qualify the addition of this therapy to the list of available surgical treatments of allergic rhinitis.
