Acute ST-Segment Elevation in Myocardial Infarction due to Paroxysmal Atrial Fibrillation- Induced Coronary Embolus in a Patient Receiving Minimally-Invasive Cardiac Surgery for Congenital Bicuspid Aortic Valve

Atrial fibrillation (AF) is a common arrhythmia, especially post-cardiac surgery. Coronary embolism (CE) is a rare complication of AF. Herein, we present the first reported case of a patient with a congenital bicuspid aortic valve with severe stenosis who underwent minimally-invasive cardiac surgery with aortic valve replacement (AVR), complicated by paroxysmal AF (PAF) and CE. We performed thrombus aspiration and injected adenosine intracoronarily via an aspiration catheter from the apical left anterior descending (LAD) artery back to the culprit lesion to achieve thrombolysis in acute myocardial infarction (TIMI) with a grade three antegrade flow. The patient then received warfarin plus clopidogrel for one month, followed by simple anticoagulant therapy with warfarin for long-term treatment.
