Probable Neuroleptic Malignant Syndrome Following Paliperidone Treatment and Recurrence During Low-Dose Quetiapine Challenge-A Case Report

We report a rare case of a 23-year-old Taiwanese female who developed neuroleptic malignant syndrome (NMS) several days after taking paliperidone, a new atypical antipsychotic, to treat a first episode of psychosis. On the fifth day, the patient developed drooling, confusion, and fever, and was brought to our emergency department. Central nervous system infection, seizure attack, and a brain lesion were ruled out by lumbar puncture, magnetic resonance imaging, and electroencephalography. In view of the symptoms, which included stupor, rigidity, hyperthermia and autonomic instability, NMS was suspected. We prescribed a low dose (25 mg) of quetiapine on the 16th and 24th days, but altered mental status, hand tremor, and muscle rigidity recurred on subsequent days. The manifestations following quetiapine use may indicate that the serotonergic pathway could play a role in NMS, and not just the D_2 dopamine receptor blockade.
