Closed Percutaneous K-Wire, Knowle's Pins Stabilization for Displaced Fractures of the Surgical Neck of the Humerus

使用穿皮骨釘固定術治療移位性肱骨頸骨折之經驗

本院骨科自1986年起,共有十五例移位性肱骨頸骨折病例使用穿皮骨釘固定術治療,八位女性、七位男性,年齡範圍從十一歲到七十二歲,平均年齡四十二歲,平均追蹤二年,沒有缺血性壞死,沒有骨折癒合不良。患者在全身麻醉下正確復位後,五位小孩使用Kirschner wire,末端用Bone cement固定。十位成人則使用有螺紋的Knowle's pin 來固定骨折。所有患者由於堅穩的固定,因此預後皆佳。使用穿皮骨釘固定術可以避免開刀復位後留下大且不雅之疤痕,因此我們逐漸使用穿皮骨釘固定術來治療移位性肱骨頸骨折。Closed Percutaneous K-Wire

For the last 25 years, many different methods of open reduction and internal fixation to repair displaced fractures of the surgical neck of the humerus have been used at Chung Shan Medical Hospital, including wire loops, metal plate, A-O T-plate, Rush pin, and screws. Since 1986, we have treated a series of 15 displaced fractures of the surgical neck of the humerus using the closed percutaneous Kirschner wire and /or Knowle's pins stabilization method. Migration of the smooth Kirschner wire was prevented by adhering the distal wires with methymethacrylate or using threaded Knowle's pins. After the reduction, we had good functional results with not complications, allowing us to strongly recommend transcutaneous fixation.
