一位護理之家住民罹患紅斑性狼瘡併失明之照護經驗
Nursing Experience of a Nursing Home Resident with Systemic Lupus Erythematosus Combining Blindness
摘要
本文描述一位44歲罹患紅斑性狼瘡併失明中年婦女於護理之家之照護經驗，護理期間自2011年1月1日至2011年6月30日，以十一項功能性健康型態為護理評估工具，藉由觀察、會談及身體評估等技巧收集資料分析後，確立個案護理問題有：如廁、沐浴自我照顧能力缺失、營養不均衡：多於身體需要、適應障礙、社交隔離、身體心像紊亂等問題。照護期間藉以關懷、支持與鼓勵，與個案建立良好的治療性人際關係，取得個案信任後，有助於啟動促進個案自我照顧模式與改善營養不均衡狀況；藉由男友與教友之可利用社會支持的協助，改善適應障礙與社交隔離等問題，多元措施協助個案適應驟變後的生活型態與身體心像紊亂，團隊合作下協助住民與家庭成員間團聚的期望，促進住民心理、社會及靈性等層面健康，重拾自信心並建立正確自我照顧方式，且以正向態與有效方法面對日後適應的生活，引導個案主動參與團體活動，適應健康狀況之改變。期望藉由此個案之照護經驗，提供此類族群長期照護之臨床照護參考。

This report describes a nursing experience of a 44 years old nursing home resident suffering from systemic lupus erythematosus (SLE) combining with blindness. The nursing period was from January. 1, 2011 to June, 30, 2011. The author assessed the resident with a Gordon eleven function health patterns framework with the skill of observations, interview, clinical care and physical assessment. After analysis the health problems were identified: 1. bathing/hygiene self-care deficit; 2. nutrition altered: more than body requirements; 3. adjustment impaired; 4. social isolation; and 5. body image disturbance. The process provided nursing care which incorporated the using empathy, companionship and encouraging attitudes results to a good relation between the patient and nurse. After gaining the trust of the patient we were able to improve her self-care abilities and also improved her condition of over nutrition. Through the help of her partner who has same religious beliefs with the patient she was able to gain social support thereby improving her inability to adapt and socialize with her surrounding environment. After diversified nursing care her body image disturbance had improved. Through the corporation of her partner and staffs her expectation 10 reunite with family was achieved. By using the nursing process, encouraging patients to voluntarily attend activities, adapting to personal health conditions, it was found this can also help resolve many difficulties and problems. It is hoped that this case study can provide reference for future clinical nursing care.
