高模擬情境模擬教學在臨床小課中的應用及經驗

摘要


目的:探討高模擬情境模擬在臨床小課中急救技能及臨終關懷護理教學中的應用及經驗。方法:通過設計臨床急救及臨終病人的護理場景,利用高模擬模擬人進行臨床情境模擬,學生參與不同角色的實踐,教師注重臨床小課中的引導、結束後的反饋及總結,課程結束後行學生無記名問卷調查。結果:問卷調查結果顯示學生對急救技能及臨終關懷護理臨床小課的滿意度為94.74% 及73.68%。學生認為高模擬情境模擬的臨床小課教學加深學生對急救及臨終關懷護理的理解,得到平時較少實踐的急救及臨終關懷護理臨床經驗;對護士角色有更深的認識;能加強自我反思、評估、團隊合作及對自我不足的審視能力。結論:高模擬情境模擬臨床小課的教學有助於縮短護理教育與臨床實務需求之差異;有助於客觀評估學生臨床學習的成效;有利於培養學生臨床評判性思維能力、解決問題的能力及團隊合作能力;同時有利於學生未來更好的更快的代入護士角色,激發學生內在學習動機,以拓展提升學生的專業能力及護理價值的建立。學生未來能將學校所學更好的應用於臨床實務中,有助於培養學生能更好的應對充滿變化的臨床情景。模擬情景訓練內容的結合需臨床設計達到更多樣性;情景的安排能讓學生更快融入;及能有更清晰的事後正確的處理,流程亦是將來此科目需改善之處。

Aim: To explore nursing student' s perceptions of high-fidelity resuscitation and end of life care simulation training in Clinical Mini-Lecture. Methods: A resuscitation scenario and an end of life care scenario were designed and presented by using a high-fidelity patient simulator. Students were allocated with different nursing roles during the simulation, while instructors mainly focused on facilitating the scenario and the debriefing section. Data was collected by inviting the participants to fill up a questionnaire regarding their satisfaction, gains and suggestions after the class. Result: Around 94.74% and 73.68% of the participants were satisfied with the resuscitation scenario and end of life care scenario respectively. It was believed the application of high-fidelity simulation in Clinical Mini-Lecture enhanced their understanding of these two topics, since these were seldom performed and practiced during clinical placement. Students also thought they had a better knowledge on various nursing roles and were enabled to reflect on their weaknesses. Both assessment and collaboration skill were also thought to be strengthened. Conclusion: The utilisation of high-fidelity simulation in Clinical Mini-Lecture helped bridging the gap between nursing education and clinical practice and enabled an objective evaluation of clinical study. Students could also be better prepared with their future nursing roles with the use of high-fidelity simulation. Moreover, they were motivated to learn and thus their professional competence and nursing values could be well-developed. Hence, students are believed to be better equipped and be able to apply their knowledge to clinical practice in the future. This subject would be further refined through intergrating the simulation scenarios training content with clinical case to make it more diversitied, facilitating the simulation arrangement so that students could integrate into the scene more quickly, and clarifying students the appropriate treatment after the simulation.
