Case Report: Allergen-Specific Immunotherapy on Canine Atopic Dermatitis Concurrent Canine Demodicosis in a Rough Collie

病例報告：減免治療於犬異位性皮膚炎併發犬毛囊蟲症之粗毛牧羊犬

摘要

一隻11月齡之未結紮雄性粗毛牧羊犬因長期皮膚問題而求診，其臨床症狀包括臉部、嘴吻部、頭部、頸背部、鼠蹊部、四肢末稍以及腳掌的潰瘍性病灶與傷口。經由過敏原皮內測試（intradermal skin test, IDST）之陽性結果確認病畜患有異位性皮膚炎（atopic dermatitis, AD），但在拔毛檢查以及皮膚搔刮檢查之後，於鏡檢中發現大量之毛囊蟲體。一般在患有毛囊蟲症的情況下並不允許以類固醇治療異位性皮膚炎，因此建議改以減免治療（allergen-specific immunotherapy, ASIT）替代。經過四個月的減免治療與毛囊蟲症治療，病畜之毛皮已完全長回，且其因異位性皮膚炎所導致的臨床症狀與搔癢情況也因此獲得緩解。

An 11-month-old, intact male Rough Collie was referred for chronic skin problems of ulcerated and excoriated lesions on face, muzzle, head, back, inguinal area, extremities and paws. Intradermal skin test (IDST) confirmed condition of atopic dermatitis (AD), but in the microscopic findings of hair pluggings and skin scrappings also revealed large amounts of Demodex mites. Treatment of AD using glucocorticoid was contraindicated in demodicosis. Therefore allergen-specific immunotherapy (ASIT) was commenced instead. Four months after ASIT and treatment for canine demodicosis, the hair coat was fully grown and the clinical signs and intensity of itchiness associated with AD also resolved.
