本文為語言治療師協助一位幼年時期因為生理結構異常,成年後因音韻異常,在求職與異性交往際遇困擾,求助治療過程。語言治療師初步進行口腔功能評估,發現個案口腔輪替動作較差,語音清晰度不足(約50%-60%),依據本院研發之國語構音評量表評估結果,個案舌根音化、鼻聲隨韻尾省略。經診斷為功能性構音及音韻異常,執行語言治療計畫包括口腔機能訓練、聽覺區辨訓練、正確語音的認識、目標語音的練習、正確語音的類化等,每週進行一次,每次30分鐘共治療20次。結果個案口腔輪替動作速率(SMRs)增加,語音清晰度提升。治療顯示,年幼期構音問題影響就學後注音符號的學習、學習態度與同儕關係,成年期則影響求職、升遷、人際關係及異性交往等,且因學齡前構音問題形成習慣性,長大後矯治費時又費力,因此建議音韻異常之語言治療越早越好。
This paper reports a case of a successful speech therapy. The case is a phonologic disorder adult who was a victim of physical disorder in his ankyloglossia. After numerous difficulties in job seeking and personal relationship, the case seeks help to the hospital. We describe the process and progress of this therapy. In the first diagnosing stage, the therapist examined the patient with Chinese Phonologic Assessment Scale that was developed by the hospital, and found that the case had problem in language articulation because of multiple dysphonic problems. The speech therapist performs a series of therapy process. Weekly therapy for 20 times, 30 minutes for each time. This is a successful case. We further suggests that problems of physical disorder, particular those dysphonia should be treated as early in the patients juvenile age as possible.
