應用模擬訓練教學設計提升新進護理師水療換藥技能之初探

Evaluation of the Effectiveness of Applying the Simulation Training to Enhance Hydrotherapy Dressing Skill among New Nurses inBurn Center

摘要

目的：由於燒燙傷之預防廣為宣導，使得燒燙意外的病人逐年減少。但若遇重大工安意外或大量火警意外，這些病人仍需進駐燒傷中心，尋求更進一步之重症治療，因此燒傷中心之設備及人員訓練，仍是醫學中心必須具備之重要訓練技能之一。面對病人數不穩定，但又必須維持熟練水療換藥技能，是燒傷中心訓練的主要目標。本研究應用「模擬訓練教學」，以增進新進護理師水療換藥之技能學習。方法：於101.6.1-102.12.30實施水療換藥模擬訓練及課程教學，總共訓練轉任或新到職單位滿3個月之護理師共15位，進行實際情境演練及課室教學與回饋。結果：參與此模擬情境演練後，其水療換藥概念成績顯著優於訓練前之成績，t=-4.090，P＜0.01，護理師學習平均滿意度為4.6分，病人對於換藥過程整體感受之平均滿意度為4.5分。臨床實務建議：利用模擬訓練教學之設計，可以提升對於特殊且重要技能學習之滿意度及病人對於照護之正向感受。

Purpose. Sincetheprevention strategies ofburns have been widely propagated, the incidence of burn injury is decreasing year by year. But the in case ofa majoraccident ora large number ofwork fireaccidents, these patients arestillstationed in theburn centerand seekfurthertreatment, so the burn centerequipmentandskill training, is still animportant. Despite the instability of patient numbers, the major goal of training in the burn center (BC) is to maintain proficient dressingskills of nurses. This study aimed to apply the ＂simulation training＂ to improve hydrotherapydressing skills of newnurses. Methods. From June 1, 2012 to December 30, 2013, 15 new nurses who entered BC for less than 3 months underwent the simulation training for hydrotherapy dressing. Attraining, these nurses experienced implementation of actual situation exercises and teaching-feedback in the classroom. Results. The nurses, after participating in this ＂simulation training＂, had significant better scores of the dressing concept than they had before training (t=- 4.090, P ＜0.01). The mean satisfaction score for the simulation training in participants was 4.6/5 points. In addition, the mean satisfaction score for the dressing process in patients was 4.5/5 points. Application of Nursing Practice. Application of the ＂simulation training＂ may enhance satisfaction for learning special and essential skills in nurses and also positive feelings for dressing in patients.
