本篇個案報告探討照護一位初次診斷為惡性淋巴瘤患者的護理經驗。個案經歷疾病的診斷與治療階段,因面臨繁複的疾病檢查與等待未知的檢查結果,出現不確定感,甚至有轉院的打算。筆者運用Gordon十一項健康功能型態做為評估架構,發現個案有下列三項健康問題:(1)焦慮/與疾病的不明確和無法預期及治療的複雜性有關、(2)知識缺失(特定的)/與對疾病及治療的資訊不足有關、(3)潛在危險性損傷/與疾病導致的白血球及血紅素降低有關。在護理過程中,筆者運用主動關懷,及早的介入照護,與個案建立信任的護病關係,提供完整的資訊,使其對疾病有正確的認知,與協助支持系統的建立,讓個案正向調適其不確定感,情緒得以抒發,避免病急亂投醫,防止延誤治療時機的遺憾。報告中並針對不確定感的概念加以討論,期能作為日後照護癌症初診斷患者經驗之參考。
This article explores nursing experience of caring a patient with newly diagnosed malignant lymphoma. The female patient underwent diagnosis and treatment stage of the disease, and because she faced complex disease inspection and test results for unknown conditions, she experienced a sense of uncertainty, and even a transfer plan. The Gordon 11-item health assessment was used to identify problems in the nursing process. Health problems of anxiety, deficiency of specified knowledge and risk for injury were identified. In the nursing process, the author used active concern, early intervention care, and established a nurse-patient relationship based on trust. Furthermore, complete information on the disease to allow correct patient cognition of the disease was provided, and a support system established, which assisted the patient in positively coping against uncertainty to help express her emotions, solve her health problems and prevent delays in treatment. The concept of uncertainty was also discussed, which is definitely helpful for patients with the same situation.
