運用羅氏理論於初診斷愛滋感染者合併骨髓炎之護理經驗

Nursing Experience of applying Peplau's Theory for a Patient with Borderline Personality Disorder

摘要


愛滋病患常面臨身、心及社會三方面壓力,本個案報告乃運用羅氏適應模式,探討一位家中決策者因骨髓炎初診斷愛滋病後,遭受許多身心衝擊及主要照顧者面對家人罹患愛滋病之震驚與照顧上憂慮之調適。護理期間為2006年7月11日至8月16日,透過身體評估、實際照護、會談等方式了解個案需求,確立有體溫過高、疼痛、身體活動功能障礙、情境性低自尊、照顧者角色緊張之護理問題。運用敏銳觀察力及評估能力,提供完善護理措施,如緩解疼痛及發燒時促進生理舒適,建立信任關係,增加個案和家屬對疾病了解,並增進照顧知識與技巧,協助他們面對此疾病衝擊,進而達到良好生活品質。期許本文能對日後護理人員照顧此類病患,提供完整的照護經驗參考。

Most Human Immunodeficiency Virus (HIV)-infected patients have stress in physiological, psychological and social aspects. By applying the Roy adaptation model, we analyzed a case (who was the main decision maker in his family) from July 11(superscript th) to Aug. 16(superscript th), 2006. After being diagnosed to have HIV infection with osteomyelitis, he not only faced physiological and psychological stress but also had to deal with the shock of his whole family. With physical examinations, clinical caring and interviews to investigate the patient's need, we confirmed the case had nursing issues including hyperthermia, pain, physical disability, low self-esteem and anxiety. Our careful observation and evaluation provided both the patient and his caregiver with considerate nursing care, such as relieving his pain and fever to restore his well-being feeling, to establish rapport and to improve his understanding of the HIV disease course and related knowledge and caring methods. All the above measures helped the patient and his family have less impact from HIV infection and have better quality of life. Our caring experience suggests comprehensive nursing care could help complicated newly diagnosed HIV-infected patients.
