增強社區精神病患正向自我概念使勝任工作之護理經驗

A Nursing Experience of Enhancing Positive Self-Concept of a Psychiatric Patient in Community to Achieve Job Competence

摘要

本文乃筆者協助一位慢性精神病患者，因長期負向自我認同影響個案生活希望、社交互動、自尊及工作適應等。藉由觀察、會談及整體性護理評估等技巧收集資料及確立問題，發現個案主要健康問題為個人認同紊亂、個人因應能力失調及社交互動障礙。筆者以為期近一年的時間，協助個案勝任其工作以增強自我認同。個案除可持續就業長達一年，其自我接受度也增加，更改變對疾病的認知、重建自信，而獨立自主的生活。此個案終能落實社區精神復健的目標，故分享此護理經驗，期待更多社區精神病患能於協助下自信自主地生活於社區中。

This paper detailed a nursing experience of assisting a chronic psychiatric patient whose hope, social interaction, self-esteem and job performance had been affected by long term negative self-identity. The author verified the patient's problems and collected data through holistic nursing assessment, observations and interviews. Three nursing problems, low self-esteem, ineffective individual coping and social interaction deficit were identified. During a nearly one-year period, the author assisted the patient in improving job competence so as to enhance her self-identity. The patient could not only sustain employment for long term one year and improve self-acceptance but also alter disease recognition, rebuild self-confidence and live independently. This nursing experience could serve as a good reference for successfully reaching the goal of community psychiatric rehabilitation. Furthermore it is expected that with assistance more psychiatric patients could live confidently and independently in the community.
