摘要

本文敘述照顧一位頑固性癲癇病患，於初次懷孕期間癲癇發作次數增加，順利生下健康女嬰之照護經驗。筆者於97.6.3至97.6.9運用高登(Gordon)十一項功能性健康評估為指引，經觀察、會談及出院電話訪談等進行資料收集，確立個案有潛在性傷害：跌倒及因抽搐導致的傷害、營養少於身體所需及無效性因應能力之健康問題。護理過程中運用會談及多次電訪建立互信之護病關係，讓個案及家屬瞭解癲癇、胎兒與懷孕相互影響之關係。護理人員也瞭解個案及家庭的照護需求、整合相關醫療資源介入提供所需照護及衛教諮詢，使其安然度過懷孕階段，順利產下健康女嬰。期望此照護經驗能引發護理人員對癲癇孕期照護的重視，並作為臨床照護之參考。

This report describes a patient with intractable epilepsy and increased seizure frequency during the initial stage of pregnancy who successfully delivered a healthy female infant. From June 3 to June 9 2008, we used the Gordon 11 functional health assessment as a guide in observation, in-hospital interview and telephone interview after discharge to collect data and establish factors of potential injuries: falls, injury caused by convulsion, nutrition less than body requirements, and ineffective response to the ability of health problems. During the course of nursing intervention, we used face-to-face and telephone interviews to build up mutual trust and establish a nurse-patient relationship in order to provide the patient and her family with more information about the interaction between epilepsy and pregnancy. Nurses also became aware of the needs of the patient and her family for being taken care of, and were able to integrate the related medical resources, provide the necessary care and health education, help them go through the pregnancy, and at last, assist the patient to give birth to a healthy baby girl. It is hoped that this personal experience will allow nursing staff to pay more attention to the care of epileptic patients during pregnancy and will also be used as a reference for clinical care.
