摘要

本研究目的在了解主要照顧者照顧精神分裂病人過程中，所經歷的烙印經驗及因應行為。運用質性研究設計，以深度訪談方式訪問15位精神分裂病人的主要照顧者，並運用主題分析法分析資料以呈現主題類別。本研究發現主要照顧者與被照顧者的互動過程中，彼此交織出「照顧歷程烙印經驗脈絡圖」，可用19個次主題建構出七個主題，來呈現主要照顧者的烙印經驗與因應行為。主要照顧者在烙印經驗方面呈現：（1）症狀干擾，亂源之本；（2）烙印枷鎖，身相隨；（3）家人患病，終生勞動；（4）未來日子，何去何從四個主題。在因應行為方面則呈現：（1）探究病情，明我心；（2）心靈悸動，勇面對；（3）用盡法子，渡難關三個主題。主要照顧者其烙印經驗與因應行為是交互並存的，期望護理人員在了解精神病人家屬的困境時，能陪伴家屬發展出適當的因應行為。

This study aimed to understand the stigma experience and coping behaviors of caregivers during the process of caring schizophrenic patients. Narrative data was obtained by in-depth interviews with fifteen caregivers and a thematic analysis method was used to analyze data. A contextual picture of ”stigma experience in caring process” was developed from the interactions between caregivers and schizophrenic patients. There are seven themes linked to this contextual picture, including 19 sub-themes. The stigma experience consisted of four themes:(1)The disturbance from patient's syndrome was the sources to the troubles;(2)The stigma bound caregivers all the times; (3) patient's illness caused extra work load to families in all their life;(4)Lack of direction and goals for the future life. The coping behaviors included three themes: (1) Caregivers recognized the illness and had a clear insight; (2) Caregivers comforted the state of mind for facing the burden; (3) Caregivers made all the efforts to overcome the difficulties. The caring process interweaved stigma experience with coping behaviors. From the result, it is expected that nurses could understand the difficult situations caregivers had faced and help them to develop appropriate coping behaviors.
