一位心臟衰竭患者行心臟再同步療法的護理經驗

摘要

本文是描述一位因心臟衰竭反覆入院的患者，並初次執行心臟再同步療法之住院照護經驗，照護期間筆者藉由身體評估、會談與出院後電話訪談等方法收集資料，並以Gordon十一項功能健康型態評估，確立個案的健康問題為心輸出量減少、活動無耐力及行心臟再同步療法之心臟衰竭患者知識增進的準備度等，並針對個案健康問題提供適當的護理措施，如協助個案及家屬了解心臟再同步療法的目的及過程，學習與心臟衰竭相關的自我照顧等，並於個案出院返家時以電話訪談方式，關懷其居家自我照顧情形，進而發現個案的症狀改善，再入院情形降低，並獲得較佳的生活品質。此個案報告將提供護理人員在照護心臟衰竭患者行心臟再同步療法時的參考。

This paper detailed the nursing experience of applying Cardiac Resynchronization Therapy (CRT) on a patient with hospital readmissions for heart failure (HF). During the nursing period, data were collected via physical assessment, interview, and telephone follow-up by using Gordon's 11 Functional Health Patterns. Major nursing problems included reduced cardiac output, activity intolerance, and poor readiness for enhanced knowledge of CRT. During the nursing process, the author assisted the patient and family members in both understanding the purpose and process of CRT and improving HF self-care abilities. The author also adopted telephone follow-up on patient's self-care behavior after discharge and found the patient's symptoms were alleviated and readmissions were reduced, which could effectively improve patient's quality of life. The paper could be a good reference for nursing staff in caring heart failure patients who receive CRT.
