減少內科加護病房護理人員針扎事件之改善專案

A Project for Reducing the Needlestick Injuries among Nurses in a Medical Intensive Care Unit

摘要


針扎事件為護理人員常見的職業傷害之一,可能因而感染到血液傳染性疾病。本單位於2005年1月至2008年3月共發生16件針扎,凸顯護理人員在執行醫護行為過程中確實出現了危險。專案小組分析16件針扎事件報告後發現造成不良現象的問題為針扎防護設備不佳、無預防針扎標準作業流程、用物未立即處理、未攜帶治療盤。經改善針扎防護設備、建立標準作業流程及執行教育訓練等三項改善措施後,在專案進行期間,針扎事件降為2件,至評值期更改善至未發生任何針扎事件。由於針扎危害風險概念包含預防、發生、處置等三階段,因此本專案強調預防的重要性,同時加強單位人員依標準作業流程來執行醫療作業,達到零針扎事件為未來目標,營造出一個安全的工作環境,以讓護理人員不會暴露於尖銳物或針頭的扎傷的危險中,並感受到職業的安全。

Needlestick injuries, which expose nurses to blood-borne pathogens, are one of the most common occupational hazards for healthcare workers. In this project, 16 needlestick incidents occurred in the unit between January 2005 and March 2008. Nursing staff were at risk of needlestick injuries. In reviewing these 16 incidents, we found all injuries were caused by needles lacking safety devices, lack of standard procedures, and failure to dispose of needles promptly or utilize a tray. Interventions included using needles with safety features, implementing standard procedures, and staff training. The number of incidents dropped to 2 during the intervention and no incidents were found during the remeasurement period. Risk assessment for needlestick injuries can follow three stages: pre-injury prevention, exposure to hazards, and post-injury management. We laid emphasis on prevention measures and implementing standard controls. The goal was to have zero incidents for needlestick or sharp injuries and to have a safe workplace for nursing staff.
