摘要

本文描述一位交通事故造成肺挫傷合併氣血胸病人，呼吸困難且有休克症狀，予以氣管內管與胸管留置，住加護病房的護理過程。在照護過程中，護理健康問題為氣體交換障礙、急性疼痛、焦慮、體溫過高，透過加護病房的護理照顧，給予適當血流動力的支持，維持生命徵象的穩定，與個案及家屬建立良好的護病關係，運用同理心、主動關懷、系統性的觀察及評估，提供合宜的護理處置，並針對心理層面的護理問題給予支持性的護理措施，協助個案減輕在加護病房中的不適應，陪伴個案及家屬渡過加護病房中的照顧。

This report describes a nursing care experience, regarding a thoracic accident contusion and hemopneumothorax patient who was sent to the ICU ward due to shock symptoms with an endotracheal tube and chest tube bilateral treatment. Patient ICU care process included gas exchange disorder, acute pain, anxiety and hyperthermia presenting as nursing health problems. However, through our applicable hemodynamic support to maintain stable vital signs, establish a good relationship with the patient and her family by using the nursing skills of empathy, attention, and systematic observational assessment to offer pain control and mental support, we were able to successfully help the patient and her family pass this difficult time at the ICU.
