一位初次診斷肝癌術後病人之照護經驗

Nursing Experience of Care for a Newly Diagnosed Liver Cancer Patient

摘要

本文描述一位初次診斷肝癌患者從等待病理檢查報告到接受疾病，進而接受手術治療。術後面對身體產生不適症狀，對疾病預後不確定感，和返家後該如何自我照顧，患者有焦慮不安等情緒表現。筆者於照護期間自2013年9月19日至9月26日，運用Gordon十一項健康功能型態為評估工具，透過訪談、觀察、傾聽及身體評估來收集資料，發現個案有「焦慮/與疾病不瞭解有關」、「疼痛/與術後傷口有關」及「知識增進準備度/與居家自我照顧有關」護理問題。護理過程中，筆者運用主動關懷與個案建立信任護病關係，提供相關疾病治療訊息，給予心裡支持與鼓勵，使其能安心接受手術，對疾病有正確的認知，讓個案回歸正常生活型態。

This article described a newly diagnosed liver cancer patient who experienced to waiting for the pathology, to accepting the disease, and then having to have an operation. The patient looked anxious and uneasy when she had uncomfortable symptoms after surgery, uncertainty for the prognosis, and unknown self-care skills after discharge. The period of nursing care was from September 19 to September 26, 2013. Using the Gordon 11 healthy functional patterns as an assessment tool, information was collected through interviews, observations, attentive listening and physical examinations. Three health issues were identified：＂anxiety/ related to disease condition＂, ＂pain/ related to surgical wound＂ and ＂readiness for enhanced knowledge/ related to home self-care＂. During the nursing process, the author provided caring and established a helping-trust relationship, presented disease-related treatment message, gave mental support and encouragement, enabled the patient to face surgery peacefully, had correct cognition to the disease, and assisted her returned to a normal life style.
