摘要

本文旨在探討一位腸造口術後嬰兒之護理經驗，筆者於2013年6月2日至2013年6月22日照護期間，運用Gordon十一項功能性健康評估工具執行護理評估，藉由直接照護、觀察、會談、儀器監視、查閱病歷等方式收集相關資料，確立個案有體溫過高、急性疼痛、照顧者角色緊張之護理問題。筆者以「創造父母賦權機會」模式提供個別性護理措施，引導照護者積極參與腸造口照護，並與照顧者共同擬訂衛教計劃及回覆示教，藉由鼓勵照顧者表達其內心感受，增進照顧者學習動機，提高學習照護之信心，降低照顧者壓力，成功提升其角色功能與照護能力，讓個案能順利返家，故將此護理經驗提供臨床兒科護理人員照護腸造口之參考，進而持續提升照護品質，以達到全人照護之目標。

This case report was a nursing experience on an infant with postoperative ileostomy from June 2nd to 22nd, 2013. We used the Gordon function health assessment guide to assess the nursing problems. Based on clinical data collected from direct nursing, observation, interview, monitoring, and chart review, the health problems included: hyperthermia, acute pain, and caregivers' role strain. We applied the COPE Program and offered individualized care for the caregivers to actively participate in the ileostomy care, customized caregivers' instructions and gave a return demonstration. This encouraged parents to express their inner feelings, meliorating the learning motivation and the nursing confidence. It also relieved their stress and advanced their role function and nursing ability, so that our patient could return home smoothly. Hence, we introduced our experience for pediatric nurses to amend the nursing quality and attain holistic nursing.
