運用雲端照護平台協助一位老年糖尿病病人血糖控制之照護經驗

摘要

本文是描述一位老年第2型糖尿病個案，運用雲端照護平台管理血糖之照護經驗。照護期間為2015年1月15日至4月30日，運用美國糖尿病衛教學會的七項自我照顧行為(AADE7 Self Care Behaviors)並藉由病歷查閱、會談、即時通訊軟體(LINE)、電子郵件等方式收集資料進行評估，發現主要健康問題為：(一)潛在危險性不穩定血糖/與血糖監測方式、次數不足及飲食行為不佳有關；(二)能力增進的準備度/與想要加入雲端專業照護平台與期望血糖控制良好有關。筆者運用雲端照護平台於護理過程中，協助個案執行血糖監測，將資料上傳雲端平台，並透過醫療團隊的即時回覆，逐步修正飲食行為、藥物的調整；邀請家人加入控糖「夥伴」，一起加入關懷個案的行列，協助個案處理問題。個案三個月後的糖化血色素由8.2%降低至7.3%。期待藉由資訊及「病友社群」的概念推廣於慢性病個案管理中。

This article described the experience of using a professional patient management platform to control blood sugar in an elderly patient with type 2 diabetes. The health care service was provided from January 15th to April 30th, 2015. The author applied AADE7 self care behaviors and collected related information by chart review, interviews, LINE, and e-mail to assess the client. The major problems identified were as follows: (1) the risk of unstable blood glucose was related to the blood glucose monitoring method chosen, insufficient monitoring and unhealthy eating habits; and (2) the level of preparedness to enhance abilities was associated with the wish to join the cloud-based health care platform and the expectation to better control blood sugar. The author made use of a cloud-based health care platform in the treatment process, and assisted the patient to monitor blood sugar and upload the information to the cloud-based platform. Through timely feedback from the medical team, unhealthy eating behaviors were corrected progressively, and medication was adjusted. Family members of the patient were invited to join the health care project as sugar control ＂partners＂, to show care for the patient and help solve problems. After three months, the patient's level of Hemoglobin A1C had decreased from 8.2% to 7.3%. It is hoped that this procedure will be promoted in the management of chronic cases through the concepts of information and ＂patient social network＂.
