導尿管相關泌尿道感染,一直是醫療照護相關感染最常見原因,亦是本單位最常見感染部位,因此成立專案旨降低導尿管相關泌尿道感染率。據統計自2014年7月起,單位導尿管相關 泌尿道感染率由千分之7逐漸上升至千分之15,經原因分析後,運用團隊合作方式擬定下列解決辦法:制定導尿管照護流程機制圖、ISBAR交班模式、規劃相關在職教育、製作「導尿管留置天數警示白板」等改善策略後,於2015年6至8月份導尿管相關泌尿道平均感染率下降達千分之4,成效顯著,將平行展開相關病房繼續執行,寄望提升導尿管照護品質。
Catheter-related urinary tract infection is the most common cause of healthcare-associated infection, and is the most common site of infection in the unit, therefore a special project was set up aiming to reduce catheter-related urinary tract infection. According to statistics since July 2014, a catheter-related urinary tract infection rate of 7 per mille gradually rose to 15 per mille in the unit, and following analysis of the reasons, the following solutions were developed by using team cooperation including development of catheter care process mechanism figures, ISBAR shift mode, planning related to job education, and making "catheterization warning whiteboard" improvement strategies. In June 2015 to August, the average rate of urinary tract-related urinary tract infection decreased by 4 per mille, and this significant effect induced a parallel commencement in relevant wards to continue to implement these solutions, in the hope of enhancing the quality of catheter care.
