中西醫結合照護一位末期腎病血液透析個案之門診護理經驗

摘要

本文主旨為運用中西醫護理措施於一位血液透析(本文中簡稱透析)併高血磷及便秘個案的護理經驗。筆者護理期間於104年5月14日至104年8月20日，護理過程中以望、聞、問、切四診做為整體性資料蒐集及護理評估，其健康問題有無望感、便秘、特定的知識缺失等，於護理過程中應用中西醫結合照護，以情志護理予以傾聽、主動關懷及支持來協助其面對長期透析、示教穴位按壓以緩解症狀及衛教腎病飲食與藥膳調護。並適時給予問題釐清等護理措施，建立其正向的態度，有效改善個案健康問題，進而提升其生活品質。運用中西醫結合照護可更深入了解個案面臨的問題，並給予個別性及符合文化性的護理措施。

The purpose of this paper is to provide the experience of nursing care in a patient with hemodialysis high blood phosphorus and constipation from 14th May to 20th August 2015. The author used the four examinations of Traditional Chinese Medicine as the comprehensive assessment methods to collect information. The health problems were hopelessness, constipation, and knowledge deficit specifically regarding the contracted disease. With the emotional care measures such as listening, active care and support, the author assisted the patient to face long-term dialysis treatment, relieved symptoms with acupressure to educate knowledge of specific diet and medicated care, clarified the health problem in a timely manner, established his positive attitude and improved the patient's health and quality of life effectively. In summary, integrated Chinese and Western medicine in the nursing process can foster in-depth understanding of the patient’s health problems and help in administering individual and cultural measures of nursing care.
