照護一位肝臟撕裂傷行肝動脈栓塞個案之急診護理經驗
Emergency Nursing Experience of a Patient with Liver Laceration Receiving Hepatic Artery Embolization
摘要
本文描述一位因車禍導致肝臟嚴重撕裂傷個案，緊急進行肝動脈栓塞術之急診護理經驗。筆者於2014年11月13日14:06~22:00護理期間，利用Gordon十一項健康功能型態為評估工具，經由觀察、會談、身體評估、直接照護、查閱病歷等方式收集資料，分析歸納個案有體液容積缺失、急性疼痛、焦慮等護理問題。因個案係屬高危險受傷機轉轉院而來，故照護期間提供個別性、整體性之護理措施，以緩解急性生理問題；一方面運用傾聽、同理心、主動關懷，鼓勵個案表達內心感受並減少其焦慮不安，另一方面協同家屬參與個案照護計劃，使個案能有信心面對其健康問題。期望藉此分享急診護理經驗，提供日後護理同仁照顧類似個案之參考。

This article describes emergency nursing experience of assisting hepatic artery embolization, which is a case of a car accident resulting in severe liver laceration. During the period (November 13, 2014 14：06~22：00), by using Gordon's 11-items functional health patterns, the writer analyzed information collected through observations, interviews, direct nursing, physical assessment, medical history, and thereby made some conclusions. The primary nursing practices included management of body fluids induction deficiency, acute pain, and body image disturbance. The patient was transferred from another hospital, with a high-risk injury mechanism. We provided individualized methods to deal with the above problems in a holistic approach. Her pain was relieved by nursing care. In addition, we attended to measures of listening, empathy, active concern, and encouraged the patient to express her feelings to ease anxiety. Furthermore, we helped her family members participate in the care plan, which enabled the patient to have the confidence to face her health problems. By sharing some emergency nursing experience in this case, the authors hope it will benefit nursing staff faced with similar cases.
