「勉強出勤」(Presenteeism)是一種工作中普遍存在的危害因子,不常被注意的負面現象,意指護理人員在出勤日的身心狀況無法負荷當日工作所需。勉強出勤會引發過勞,亦可能造成醫療疏失或危害病人風險的結果。國內外解釋的不同與中文譯名的不一致,為更加瞭解勉強出勤的概念,本文依據Walker及Avant(2011)步驟進行勉強出勤之概念分析,藉此探討概念的定義、定義性特徵、前因、後果並與相關概念區別,並透過不同類型之案例,釐清概念、確認勉強出勤之前置因子與後果。透過提供護理人員審視勉強出勤的現象,本文提醒過度付出自我照顧他人之護理人員需注意自身現象,護理主管需妥適處理。安全是護理的核心工作價值,惟有安全出勤的護理人員才有安心的護理照護使病人安適的關懷。
"Presenteeism" is a kind of hazardous factor prevalent in the workplace, and it is also a negative phenomenon which is not paid adequate attention to. It means that the physical and mental condition of the nurses cannot afford the workload of attendance, possibly leading to overwork in nurses, malpractice, or life-threatening risks in patients. Since there are different interpretations by foreign and Chinese scholars, we conducted a concept analysis of presenteeism using the concept analysis method proposed by Walker and Avant (2011) to have a better understanding of the concept. By doing so, we explored the definitions, attributes, antecedents, and consequences of presenteeism. Through different types of examples, we attempted to clarify the concept and confirm the antecedents and consequences. This article aims to help nurses check whether presenteeism occurs on themselves, reminds them not to overwork while caring for patients, and also reminds nursing supervisors to deal with this negative phenomenon. Safety is the core value of nursing, and only when attending without presenteeism, can nurses provide secure and comfortable care for patients.
