運用Watson關懷照護理論於一位學齡期大腸癌個案之手術全期護理
Application of Watson's Caring Theory to Perioperative Nursing: A Case Report of a School-aged Child with Colorectal Cancer
摘要
內文旨在協助一位10歲學齡期兒童，因大腸癌入院行右側結腸根治性腸手術併鎖骨下靜脈輸注管套植入手術之全期護理經驗。自2017年6月8日至6月19日護理期間，藉由術前術後訪視、觀察、會談、病歷查閱及身體評估等方式收集資料，分析歸納確立個案有恐懼、急性疼痛、潛在危險性傷害及案母有照顧者角色緊張的健康問題。護理過程以Watson關懷照護因素結合手術全期護理理論，且根據學齡期發展特性，運用「漫畫一般外科手術」一書，透過輕鬆有趣的引導讓個案了解手術過程，增進手術認知緩和恐懼；術中維護手術安全；術後緩解傷口疼痛，並提供主要照顧者相關疾病資訊及照護技能，以減緩焦慮提升照護者自信。

This essay describes all the caring modalities of colorectal cancer surgery and adjuvant chemotherapy for a 10-year-old school child. Data of the patient was collected from June 8th, 2017 to June 19th, 2017 by interviews, observation, chart review, and physical examinations, and the data revealed issues with the surgery including the patient's fear for operation, acute pain, potential harm, and insufficient care from the patient's mother after surgery. The whole process of patient care was given according to Watson's caring theory and perioperative nursing. To further accompany the patient's age, humorous ideas from a comic book ＂General Surgery Comic＂ were also included so that the patient could know more about the operation. In summary, our perioperative nursing process enhanced surgery safety, post-operative wound pain relief and post-operative care acknowledgement along with confidence-building for the patient's mother.
