背景:臂神經叢損傷多由車禍造成,造成上肢功能障礙,影響生活品質,甚至造成憂鬱。目的:本研究主要目的為探討臂神經叢損傷病人上肢功能、生活品質及憂鬱的相關因素。方法:橫斷式研究,以結構性問卷進行收集,於2014年11月至2015年10月收案43位臂神經叢損傷術後住院病人。結果:個案年輕男性、單身、高中職教育、無工作。主要受傷原因為車禍,全臂型損傷為常見受傷型態,受傷時間小於1年者居多。上肢功能中重度失能;生活品質中等程度;多數病人有憂鬱情形。已婚與受傷時間短的上肢功能症狀較嚴重。上肢功能愈好,生活品質愈佳。上肢功能症狀愈嚴重,憂鬱比例愈高。結論:臂神經叢損傷病人的上肢功能與症狀對其生活品質與憂鬱影響重大。
Background: Brachial plexus injuries (BPI) are commonly caused by traffic accidents, resulting in disability in the upper-extremities, poor quality of life (QOL), or even depressive symptoms. Purposes: The purpose of this study was to assess the physical function in upper-extremity disability, QOL, and depression in patients with BPI, and factors associated with them. Method: This was a cross-sectional study, employed structured questionnaires as a method of data collection. Study participants were hospitalized patients who had received branchial plexus surgery. A total of 43 BPI patients were recruited between November 2014 and October 2015 from a teaching hospital in southern Taiwan. Results: The majority of participants were young males, single, with high school education and currently not working. Traffic accidents were the most common cause of BPI; participants primarily had a total arm type of brachial plexus palsy and the time since their injuries was often less than one year. They reported that the physical function for upper extremity injury lay between moderate and severe level of disability; QOL was at moderate level; and depressive symptoms were present in many cases. Study results showed that married status and time since the injury of less than one year were associated with severe levels of physical symptoms in the upper extremities; improved levels of physical function for these injuries were associated with increased degrees of QOL; while physical symptoms from such injuries were associated with depressive symptoms. Conclusion: Physical function and symptoms in upper extremity injuries play an important role for QOL and depressive symptoms in BPI patients.
