一位急性腦中風病人復健治療之照護經驗

Nursing Experience of Caring for an Acute Stroke Patient with Rehabilitation Therapy

摘要

本文探討一位老年女性因急性左側大腦動脈狹窄導致右側偏癱，因失落衍生無力感之照護經驗。照護期間為2018年3月17日至6月1日，運用Gordon十一項健康功能評估進行評估，發現個案出現身體活動功能障礙、知識缺失及無力感等健康問題。護理過程中發現病人日常生活需要協助，建議個案及案夫共同擬定復健目標；提供心理支持與鼓勵，經由家人的支持及陪伴，增強其恢復信心；透過會診營養科、社工及出院準備服務，指導糖尿病飲食及高血糖症狀注意事項，讓病人即早恢復正常功能。希望藉此經驗分享提供護理同仁面對腦中風病人衍生之身、心問題，協助病人度過疾病適應期及學習情緒管理，以做為臨床照護之參考。

This case report describes the nursing experience for an elderly female with hopelessness and self-care deficit resulting from left limb weakness after stroke following acute middle and anterior cerebral artery stenosis. The nursing period was from March 17 to June 1, 2018. The method of Gordon's Eleven Functional Health patterns was applied to assess the patient's condition. Three health-related problems were identified: physical activity dysfunction, knowledge deficit, and hopelessness. Nursing interventions including providing physical and occupational therapy to improve the muscle power and assist in daily life; setting the rehabilitation goals with the patient and her husband; providing psychological support, encouragement, and strengthen rehabilitation through family support; and educating the homecare safety and diabetes diet precautions by the professional team of the nutritionist, social worker and discharge planning nurse were applied to the patient to return to normal function as soon as possible and reduce the risk of secondary stroke. In addition, we also helped the patient to adjust to the disease adaptation period to learn emotional control and to promote post-stroke self-care. Finally, we wish to share this experience for nursing professionals who are taking care of stroke patients with physical, psychological, and spiritual problems.
