本文為協助一位直腸癌末期病人正向面對死亡之照護經驗,護理期間為2018/12/19至2019/01/12,藉由觀察、會談及身體評估與查閱病歷等方式收集個案資料,運用生理、心理、社會及靈性層面進行整體評估。在照護過程中,發現病人有營養少於身體所需、慢性疼痛以及死亡焦慮等健康問題,故筆者與營養師一同討論製作個案專屬的食物熱量表及點餐本,來增加個案進食慾望;利用熱敷及穴位按摩減緩腹脹不適感,藉由安寧療護的四道人生理念、宗教力量和家人陪伴下,開導個案以平靜的心看待生與死,使其獲得心靈的安適,勇敢面對死亡的到來,希望可以藉此照護經驗分享,讓臨床護理人員在照顧癌末病人時,在緩解癌末惡病質的不適之餘,還能鼓勵病人正向面對死亡,達到生死兩相安的善終目標。
This article discusses the nursing experience of assisting terminal patients with rectal cancer to face death positively. The duration of nursing care was from December 19, 2018 to January 12, 2019. The author collected patient data through observation, interviews, physical assessment and medical records, and then used physiological, psychological, social and spiritual aspects for holistic assessment. During the nursing process, various health problems such as nutritional imbalance less than body requirements, chronic pain and death anxiety were identified. Accordingly, the author requested that the dietitian make a case-specific food calorie table and menu with food photos to increase personal dietary desires, and used hot packing and acupressure to reduce the discomfort of abdominal distension. Being accompanied by the hospice of love, sorry, thanks and farewell and religion enlightened the patient to face life and death with a quiet heart and achieve peace of mind. Finally, in being accompanied with family members, she bravely faced the coming of death. It is hoped that this sharing of nursing experience will enable clinical nurses to take care of patients with advanced cancer, to not only reduce the discomfort of advanced cancer cachexia but also encourage patients to face death and achieve the goals of life and death.
