摘要

本文描述一位三十六歲男性福爾尼埃氏壞疽之個案，歷經多次清創手術，導致陰囊至肛門有大範圍的傷口及面臨劇烈疼痛之護理經驗。於2018年5月7日至6月1日，透過觀察、會談、病歷回溯及身體評估等方式收集資料，運用生理、心理、社會及靈性層面進行整體性評估，發現個案有急性疼痛、現存性感染及身體心像紊亂之護理問題。照護期間運用傾聽、陪伴及主動關懷之方式，建立良好的治療性人際關係，提供個別性護理措施，包括：協助個案減輕疼痛及促進傷口癒合照護等，並不斷的給予支持及鼓勵，幫助個案重建正向之身體心像，讓個案重拾以往外向開朗的自信心，最終回歸工作崗位維持社交活動以提升生活品質。

This nursing case focused on a 36-year-old male patient who suffered from Fournier' s Gangrene and underwent multiple debridement surgery. As a result, he had a wide range of wounds from the scrotum to the anus. The nursing care period was from May 7 to June 1, 2018. During this period, the author collected patient information through observations, conversations, and medical reports while assessing that the patient had multiple health problems like acute pain, actual infection, and body image disturbance. In this nursing care process, two methods were used to restore the patient's normal condition and positive body image. The first method was through a physical modality and the other was psychological. In the physical approach, the author used nursing techniques to provide individual care to the patient and reduced the physical pain. Psychologically, the author listened, accompanied and encouraged the patient to reconstruct a positive body image. Eventually, the patient regained confidence and recovered. Furthermore, he returned to work to maintain social activities and improve the quality of life.
