一位高血糖合併右臉蜂窩性組織炎個案之護理經驗
Nursing Experience of a Patient with Cellulitis of the Right Face and Hyperglycemia
摘要
本文中個案因右臉蜂窩性組織炎入院抗生素治療，同時發現有高血糖的問題，在抗生素治療後右臉腫脹未明顯改善，與膿腫形成有關，須接受清創術，進而下定決心做好糖尿病自我管理之護理過程。護理期間為2017年10月6日至10月26日，透過觀察、會談與護理照護等方式進行資料收集，運用Gordon十一項功能性型態評估，確立個案有急性疼痛、術前焦慮以及知識增進的準備度等健康問題，主動關懷並瞭解個案擔憂和需求，建立良好護病關係，針對健康問題給予合適的護理措施，並依照美國糖尿病衛教學會的七項自我照護行為，引導個案糖尿病自我管理，住院當中飲食控制與胰島素注射，血糖值逐漸穩定，使個案更有信心面對及因應，學習與糖尿病共存。

In this study, a patient with cellulitis on the right side of his face hospitalized to undergo antibiotic therapy was incidentally discovered to have hyperglycemia. After antibiotic treatment, the patient's facial cellulitis did not improve due to abscess formation, and debridement was required; therefore, he decided to go with diabetes self-management, which lasted from October 6 to October 26, 2017. Data collection was achieved by means of observations, interviews and nursing care plans, with Gordon's 11 functional health patterns being used for assessment. After determining that the patient had health problems such as acute pain, preoperative anxiety and lack of readiness for enhancing his knowledge, the researchers took the initiative to understand the concerns and needs of the patient, established a good nurse-patient relationship and provided appropriate care measures according to the patient's health problems. Additionally, the American Association of Diabetes Educators' AADE7 Self-Care Behaviors framework was adopted to assist the patient in his self-management. Following the implementation of diet control and insulin injection measures during the patient's hospital stay, his blood sugar level gradually stabilized, which increased his confidence to overcome, adapt to, and learn the ways to cope with diabetes.
