運用跨領域團隊資源管理提升手術部位感染組合式照護執行完整率

Use TRM to improve the complete rate of combined care for SSI Bundle Care

摘要


背景:外科手術感染是醫療最棘手的問題,本院雖有參與手術部位感染先驅計畫,但同仁們仍對於執行內容流程未落實,因而引發專案改善動機。目的:運用跨領域團隊照護提昇手術部位感染組合式照護執行完整率。方法:專案期間為2018年9月1日至2019年8月30日,經調查發現執行完整率低之原因為不清楚負責項目、缺乏提醒機制、照護流程不清、未開立醫囑與抗生素給予超過指引建議。透過推動跨團隊會議、修訂表單及照護流程、舉辦教育訓練與資訊系統建置進行改善。結果:手術部位感染之組合式照護執行完整率由54%提升至98.27%。結論:本專案在運用團隊資源管理後,確實達成提升SSI組合式照護的執行完整率,進而提升手術病患照顧品質及保障病人安全。

Background: Surgical infection is the most intractable issue in medical care. Although our hospital has participated in the pioneer program of SSIS, our colleagues have still not introduced the implementation process of the content, thus leading to the motivation of project improvement. Purpose: The purpose is to use cross-field team care to improve the execution integrity rate of combined care for SSIS. Methods: The investigation found that the root causes for the low execution integrity rate were unclear responsible projects, lack of reminder mechanism, unclear care procedures, no medical advice and too many antibiotics administered beyond guidelines and recommendations. Improvement was made by facilitating cross-team meetings, revising forms and care processes, organizing educational training and building information systems. Results: The completion rate of combined care for SIS increased from 54% to 98.27%. Conclusions: After using team resource management, this project has indeed achieved improvement of the implementation rate of SSI combined care, thus improving the quality of surgical patient care and ensuring patient safety.
