本專案旨在提升主要照護者對多重抗藥性菌種防護執行正確率,避免群聚感染。本單位為腫瘤相關病房,病人多接受抗生素或化學治療、免疫力低且住院天數長,為多重抗藥性菌種好發族群,臨床上觀察到家屬為落實防護措施感到緊張不安,故深入探討,發現家屬對多重抗藥性菌種防護執行正確率僅51.8%。主要問題:護理師認知不足與衛教技巧不一致、主要照護者的認知與照護技能不足、教育訓練不足、照護標準文件未整合及缺多國語言衛教單張。故研擬:整合多重抗藥性菌種照護標準、設計多重抗藥性菌種防護措施執行查核表、設計不同類別防護措施提醒小卡、運用趣味、互動性衛教工具等,使家屬對多重抗藥性菌種防護執行正確率51.8%提升90.2%,達專案目標。
The goal of this project is to improve primary caregivers' accuracy of applying protection on multidrug-resistant organisms (MDROs), thus preventing cluster infections. It is a tumor-related ward, where patients are elderly and receiving antibiotic treatment, chemotherapy, low immunity and long hospital stay while being at high risk of MDROs; according to the author's clinical observation, family were worried that they may fail to implement the preventive measures for MDROs correctly. The author then conducted an in-depth investigation and revealed that only 51.8% of primary caregivers applied health education on MDROs accurately. According to an analysis on the current MDROs protection, the main problems were as follows: nurses' insufficient knowledge and inconsistent health education skills; primary caregivers' insufficient knowledge, care skills, and educational training; lack of integrated standard care documents; and failure to provide multilingual health education leaflets. Therefore, the author proposed the following countermeasures: establish integrated care standards for MDROs; design a preventive measure checklists for MDROs; design reminder cards for different types of preventive measures; and adopt playful and interactive health education tools. These countermeasures improved the accuracy of the family application of health education from 51.8% to 90.2%, which met the project's goal.
