筆者於2018年09月13日至10月04日照顧期間運用歐倫自我照顧理論,搭配觀察、會談、身體評估、查閱病歷資料及家屬互動等方式,確立個案有身體活動功能障礙、自我照顧能力缺失、無力感等健康問題。依據健康問題提供個別性照護。當意外發生後,生活面臨重大改變,無法接受病情發展,產生抗拒心理,不願接受復健運動,故引發筆者想介入照護的動機。針對心理問題,主動陪伴並關懷,鼓勵表達內心感受,支持系統介入,協助體認自身價值,以正向心態接受復健;另外擬定復健計畫,教導運動方式,協助執行轉位技巧及輔具使用,配合反覆自我練習,以及家人支持陪伴。建議臨床照顧能多傾聽重視個案心理問題,即時給予關懷照護,協助早日走出陰霾,重新恢復生活。
During September 13th to October 4th, 2018, the author found that a spinal cord injury patient had various health problems such as physical dysfunction, inability to care for himself and muscle weakness. We applied Orem's theory combined with observations, interviews, physical examinations, medical records evaluation, and family interaction to validate these, with personal care being provided individually. When accidents happen, life changes dramatically, and many patients are unable to accept the fact, then develop unwillingness and refusal to receive rehabilitation programs. This triggered the author's motivation to help such patients. Active and intense caring is recommended to deal with psychological problems, and we encourage patients to express their feelings directly. We help them to recognize their own values and develop positive attitudes by means of supportive systems; additionally, rehabilitation plans are made to teach them how to exercise, help them how to turnover and how to use assistive devices. Repetitive self-practice and family support are also required. Clinical caretakers should listen to psychological problems as expressed by patients and provide instant care to help them regain their normal life as soon as possible.
