一位肝癌末期病人無望感之照護經驗
Nursing Experience of Hopelessness in a Terminal Liver Cancer Patient
摘要
本文探討一位肝癌末期病人治療效果不佳，反覆入院產生無望感之照護經驗。筆者於2019年5月22日至6月5日以Gordon十一項功能性健康型態評估，確立有疼痛、體液容積過多、營養少於身體所需及無望感之健康問題。照護措施包含：（一）背部按摩、藥物及音樂轉移疼痛感受；（二）提供飲食訊息及舒適臥位，改善體液容積過多；（三）了解食慾不佳原因並設計營養計畫表，增加飲食意願及攝入熱量；（四）探討病人對疾病想法，引導思考人生意義，協助適應疾病並完成願望。個案曾表達對人生失去希望，甚至影響日常生活，建議臨床照護此類個案，盡早介入跨團隊照護，提供相關資源，維持身體舒適及心靈照護。希冀本文照護經驗能提供同仁對肝癌末期病人之全人照護。

This study investigated the care experience of a patient with terminal liver cancer whose treatment was ineffective and who was experiencing a sense of hopelessness because of the need to be repeatedly hospitalized. A health assessment was performed from May 22 to June 5, 2019 using Gordon's 11 functional health patterns to assess the patient's pain, excessive body fluids, low nutrition situation (i.e., nutrition being lower than that required by the body), and sense of hopelessness. The care measures adopted included (a) applying back massages, medications and music to mitigate the patient's pain; (b) providing dietary information and helping the patient lie comfortably and prevent fluid overload; (c) identifying the causes of the patient's poor appetite and designing nutrition plans to increase the patient's calories intake and desire to eat; and (d) examining the patient's thoughts on the disease, guiding the patient in finding the meaning of life, and assisting the patient in adapting to the disease and fulfilling the patient's dreams. The patient initially expressed losing hope in life and experiencing difficulties in performing daily activities. Thus, this study introduced interdepartmental collaboration to soothe the physical and spiritual pain experienced by the patient and also fulfill the patient's dreams. The nursing experience in this study could serve as a reference to medical personnel when providing holistic care to patients with terminal liver cancer.
