照護一位燒炭自殺一氧化碳中毒個案之急診護理經驗

本文探討一位因失業及債務問題，而燒炭自殺導致一氧化碳中毒病人，照護期間為2020/8/2 23:55至8/7 07:30，以Gordon十一項健康功能型態評估，確立個案護理問題為氣體交換障礙、急性疼痛、皮膚完整性受損、無望感。照護期間給予高濃度氧氣及高壓氧治療，傷口以無菌技術換藥，透過護理照護緩解個案傷口疼痛的不適感，以主動關懷的方式，與個案建立良好的護病關係，鼓勵個案表達內心感受，聆聽其照護需求，並給予心理支持，引導其面對自殺原因，建立正向思考，增加個案自信心，出院後能回歸家庭適應社會生活，期望藉此經驗分享，能作為護理同仁往後照護此類病人之參考。

This article describes about a patient suffered from carbon monoxide poisoning who committed suicide by charcoal burning due to unemployment and debt problems. The period of nursing care was from 2020/08/02 23:55 to 08/07 07:30. According to Gordon 11 functional health patterns, individual care issues were established as impaired gas exchange, acute pain, impaired skin integrity, and hopelessness. During the nursing care, high-concentration oxygen and hyperbaric oxygen therapy were given, and the wound was cared with aseptic technique. The discomfort from the wound was relieved via nursing care. With the proactive care, we establish a reliable relationship with the patient, encourage the patient to express his feelings, listen to his requirement. We also provided psychological support, guided the patient to face the cause of suicide, established positive thinking, and enhanced his confidence. Finally, he can return to his family and adapt to social life after discharge. This nursing experience can be used as a reference for nursing staff to take care of such patients in the future.
