背景:本院2018年糖尿病共同照護網收案率54.8%,低於健保署區域醫院指標值,故引發專案改善動機。目的:提升糖尿病共同照護網收案率。方法:專案期間為2019/5/1-2020/1/31,調查導因歸類出病人方面:等門診太久不想再花時間被衛教、不知道要接受糖尿病收案衛教;醫護人員方面:單位未定期宣導糖尿病收案流程;環境設備方面:電腦點選收案操作複雜、收案無相關提示、糖尿病衛教室位置偏遠且標示不清,透過運用宣導、修訂糖尿病收案流程、增設糖尿病衛教室及標示指引、改善電腦點選步驟等對策進行改善。結果:糖尿病共同照護網收案率由54.8%提升至68.3%。結論:本專案運用團隊合作與對策實施,達到收案率提升,顯示成效良好,可平行推展至他科。
Background: In 2018, the participation rate of the Diabetes Shared Care Network in our hospital was 54.8%, which was lower than the performance evaluation indicator among the regional hospital of the National Health Insurance Administration. Therefore, motivation of project improvement is proposed. Purpose: Increase the participation rate of Diabetes Shared Care Network. Methods: The duration of this project was from May, 2019 to January, 2020. The investigation attributed to multiple aspects. Patients aspect: Patients wait for a long period of time at the outpatient clinic and hence, they don't want to spend more time on health education. In addition, some patients did not receive the proper information that they need to receive health education for their diabetes; Medical staff aspect: The unit does not regularly publicize the process of accepting patients to the Diabetes Shared Care Network ; Equipment aspect: The operation of computer selecting and accepting suitable cases into the system is complicated, and there are no applicable reminder to notify the medical staffs to enroll the cases. The location of the diabetic education room is distant, and the sign is unclear. As a result, we advocate standard procedures and revise the process to enroll patients to the Diabetes Shared Care Network, increase clear signs that can guide to the diabetic education room and improve our computer clicks and selection procedures to improve our overall participation rates. Results: The participation rate of the Diabetes Shared Care Network increased from 54.8% to 68.3%. Conclusions: The use of team cooperation in this project has indeed achieved an increased number of percentages of the participation rate, showing that our measures are effective and can be extended to other departments in parallel.
