運用多元策略提升脊椎手術病人術後復健運動執行率

Using Multiple Strategies to Improve the Implementation Rate of Postoperative Rehabilitation in Spinal Surgery Patients

摘要


脊椎退化性疾病手術後早期復健運動可有效減緩疼痛、改善病人活動功能。2020年1月至2月實際觀察病人術後復健運動情形,發現復健運動執行率僅53.0%,引發專案改善動機。現況分析發現問題有:病人傷口疼痛及疲憊不想運動、復健運動內容太多記不住、護理人員對復健運動認知不足、僅口頭教導而未實際動作示範、缺乏術後復健運動照護流程、未定期舉辦教育訓練及護理指導工具不合適等。藉由舉辦脊椎術後復健運動課程教育與技能學習、製作下肢復健運動影片、修訂護理指導單張、疼痛控制處置及運用日常管理手法等多元策略後,病人復健運動執行率提升至90.1%。期望經由持續的專案改善,提升病人復健運動成效、增進病人自我照護能力以提升生活品質。

Early rehabilitation following degenerative spinal disease surgeries can effectively relieve pain and improve patient mobility. The observed implementation rate of postoperative rehabilitation from January to February 2020 was 53.0% and must be improved further. Based on the analysis, the current situation and problems include: patients' incision pain, fatigue, and reluctance to exercise; the challenge of recalling many rehabilitation procedures; the lack of rehabilitation awareness of the nursing staff, demonstration alongside verbal instruction, standard postoperative rehabilitation process, regular education and training for staff, and appropriate of nursing instruction tools. After executing multiple strategies such as education and skill learning of spinal surgery rehabilitation, the production of a lower limb rehabilitation video, the revision of nursing instruction leaflets, pain control, and the implementation of a daily management system, the rehabilitation implementation rate increased to 90.1%. We hope that through continuous efforts, the effectiveness of patients' rehabilitation, self-care ability, and quality of life will be improved.
